Immunizations

  • Alabama requires all students to have updated required immunizations on file. Please submit updated vaccination records no later than September 1 to avoid any interruption in school attendance.

Medications at School

  • If your child needs to take medication during the school day, please bring the medication in its original labeled container along with a completed Medication Administration Authorization form signed by both the parent/guardian and healthcare provider. Please see your campus nurse for the required form.

Action Plans

  • If your child has a chronic medical condition such as asthma, diabetes, seizures, severe allergies, or another health condition, please provide an updated physician-signed action plan each calendar school year so we can provide the appropriate care. We will complete and Individual Healthcare Plan on your student per medical providers orders.

Keep Us Informed

  • Please notify the school nurse of any new medical diagnoses, allergies, surgeries, medications, or other health concerns. Keeping your child’s health information current helps us ensure a safe and successful school year.

Care coordination in schools involves school nurses organizing the care of students by sharing information and maintaining communication among those concerned with the needs and care of students with chronic health conditions (e.g., asthma, diabetes, epilepsy). This integrated approach may include:

  • Families
  • Health care providers
  • Teachers
  • Other school staff (e.g., administration)

Care coordination can provide students with chronic health conditions with the following benefits:

  • Improve medical management
  • Provide more detailed follow-up and linkages to other services
  • Reveal the need for additional resources (e.g., eligibility and enrollment assistance for health insurance)

Some students may have an Individualized Education Plan (IEP), or an Individualized Health Plan (IHP) that helps them to learn in an environment that can support their needs. Typically the school nurse plays an important role in establishing these plans.
